Our affordable memory care neighbourhood offers a simplified approach tailored to individuals with mild cognitive impairment. Our focus is on providing a safe and supportive environment that promotes daily living skills and independence. With personalized care plans and engaging activities, we help residents maintain a sense of purpose and well-being.

Personalized Care With a Community Feel

Seniors with a limited budget that aren’t quite ready for long-term care and have very early stages of dementia or mild cognitive impairment, are the perfect candidate for the ‘My Home’ neighbourhood!
Enjoy the comfort of a private, lockable bedroom, some even equipped with a sink and vanity for added convenience. Our shared bathrooms are also designed with your comfort and safety in mind - spacious, private, and easily accessible.
Enjoy the peace of mind of knowing that your loved one is safe and cared for while our emergency call system provides an added layer of security, ensuring help is always within reach.

Dining & Nutrition

Savor home-cooked meals and snacks tailored to your dietary needs and preferences. The central dining and bistro area provides a warm and inviting atmosphere for social interaction. The ‘My Home’ neighbourhood is equipped to provide a hardy continental breakfast 6 days a week plus a hot breakfast on Sunday’s. Your main ‘hot’ meal with two choices is served at lunch and dinner is a lighter option with a variety of soups and sandwiches.

Say goodbye to chores and enjoy the freedom to focus on what truly matters. Our dedicated staff takes care of the cleaning and laundry, allowing your loved one to relax and enjoy their space. Weekly housekeeping and linens are done for you, as well as twice a week garbage collection!

My Home Neighbourhood Service Package Details

At My Home, we believe in fostering a sense of belonging and purpose, even as memory fades. This neighbourhood offers a flexible solution for families looking for memory care for their loved one with mild cognitive impairment. In My Home, Forest Valley works closely with Ontario Home Health to provide its residents with personal care needs like AM/PM care or bathing if needed and qualify. Included in your package is a My Home Care Coordinator that will assist you and your family with coordinating HCCSS services and agencies that maybe needed for personal care.

Three Meals per Day

Up to four incontinence briefs are provided per resident per day, if required.

Co-ordination with Ontario Home Health

If personal care is needed by the resident, It will be provided by Ontario Home Health Services, if qualified for such services.
*Assistance with changing briefs or toileting is not available on the My Home neighborhood but may be provided by Ontario Health at Home if qualified.
